A new approach to dementia care

We’ve seen how families are left to cope on their own after a loved one finally receives a dementia diagnosis. Central Minnesota Dementia Community Action Network (D-CAN) offers a different approach through our Dementia Resource Center Clinic located in St. Cloud, MN. We focus on the patient by collaborating with their caregivers and families, their dementia services providers, and their dementia clinicians in an ongoing effort to improve care and quality of life. With this new model of dementia care, there can be “more life to live.”

D-CAN’s Dementia Resource Center Clinic is looking to hire two half-time or one full-time nurse practitioner or physician associate (PA) to see clients in our dementia clinic for both dementia and for dementia prevention evaluations.
