D-CAN has closed as of June 30, 2025

Although D-CAN officially closed its doors on June 30, we want to make you aware of similar services available through Vivie, a nonprofit organization with a shared mission of providing compassionate care. Vivie serves communities across three states and has a local office in Waite Park near St. Cloud.

Vivie offers dementia-related services through its GUIDE and Connected Communities programs—both designed to promote wellness, independence, and meaningful connection. These programs reflect many of the same values and approaches you experienced through D-CAN.

We’re also pleased to share that Tami Kolbinger and Cristina Rodriguez, former D-CAN employees, have joined Vivie and continue to serve individuals and families in need of dementia care.

If you’d like to receive Vive’s services and want help making your transition simple from D-CAN to Vivie you have two options, you can:

1. Complete this Vivie Release of Information Form that allows us to securely transfer your records to Vivie, or
2. Contact Vivie directly to begin services on your own, if you prefer.

You can return the completed form in the enclosed envelope or email it to PHTeam@vivie.org. Please ensure your email is encrypted by selecting the encrypted option in your email.

A new approach to dementia care

We've seen how families are left to cope on their own after a loved one finally receives a dementia diagnosis. Central Minnesota Dementia Community Action Network (D-CAN) offers a different approach through our Dementia Resource Center Clinic located in St. Cloud, MN. We focus on the patient by collaborating with their caregivers and families, their dementia services providers, and their dementia clinicians in an ongoing effort to improve care and quality of life. With this new model of dementia care, there can be “more life to live.”
