Clinic Practice Manager

Central Minnesota Dementia Community Action Network (D-CAN) in St. Cloud, Minnesota, is seeking candidates to apply for a new position as Clinic Practice Manager of our D-CAN nonprofit’s Dementia Resource Center Clinic (DRCC).

This is an exceptional opportunity for a skilled applicant to lead a fast-developing clinical practice whose mission is to improve access to quality dementia care in our community (Central Minnesota). With one physician, two care navigators, a community health worker, and 2 fully engaged office volunteers working amid a horizontal-based collaborative culture, we are ready for the right person with strong interpersonal, leadership, and management skills to grow our expanding practice and services as we scale up our operational and educational offerings. The ideal candidate would have a preferred degree in Social Work, Occupational Therapy, or Nursing, also leadership and management experience preferably in a medical setting with strong communication ability. Advanced Practice Clinician

D-CAN’s Dementia Resource Center Clinic is looking to hire two half-time or one full-time nurse practitioner or physician associate (PA) to see clients in our dementia clinic for both dementia and for dementia prevention evaluations. Our mission is to “improve access to quality dementia care in our community”.  Additional training will be provided by our staff physician and care navigators during onboarding as needed.  Our clinic services are highly rated by our clients.  We assess dementia risk factors with a thorough clinical evaluation and records review, then work with clients and caregivers to create a risk reduction care plan that is shared with referring clinicians along with several clinical recommendations.

An ideal candidate for this position would have 5+ years of internal medicine or primary care experience dealing with adult patients and would be open to our innovative new model of dementia care beyond just standard dementia care. Experience working with dementia patients and a passion for quality dementia care preferred.  Prospective applicants would be welcome to attend several of our clinical encounters to learn more about our care model and to be sure this type of practice is compatible with their career plans before formal application. Our DRCC is poised to scale up provision of these and other client and caregiver services in what is known as a comprehensive dementia care model. We are looking to hire clinicians with excellent communication skills who can embrace a team-oriented work culture and maintain warm and quality patient relationships as we continue our growth in Central Minnesota.
