Frontline Heroes

Recognizing frontline workers in long-term care for their personal courage and dedication to helping others

Nominate a Frontline Hero

Let’s recognize frontline care workers in Central Minnesota!

D-CAN’s Frontline Heroes Award Program focuses specifically on frontline workers such as CNA’s, home health aides, nurses, LPN’s, chaplains, and others working within long-term care facilities including hospitals, assisted living, and memory care. Individuals who have shown outstanding commitment and care to patients may be nominated. Nominations will be reviewed by D-CAN staff, and those selected will be notified prior to the announcement. Winners will receive an award certificate, and be recognized on D-CAN’s website and social media sites.

Employers can submit a nomination for staff members, and patients and/or families of patients can nominate a care worker who has touched their lives.
